To protect yourself legally and to protect your customers' health, BIOTIC Phocea has developed a health questionnaire. It is a non-exhaustive list of the most important questions. By using this questionnaire you will know when to accept or refuse to do a pigmentation treatment (the questionnaire and responses were written by doctor).
